In an effort to better inform our patients of their financial obligations, we have implemented the following policies. Please read all the information below. Your cooperation in following these policies allows us to better serve you.
Accepted Insurance Plans
Great Lakes Ear, Nose and Throat Specialists accepts most major insurance plans. This makes it convenient for patients and referring physicians. Accepting most plans allows you to keep the same doctor even if your health insurance changes.
Some insurance plans require the patient to obtain a written or electronic referral from their primary care physician prior to a consultation. It is the patient’s responsibility to make sure the appropriate arrangements have been made. Please discuss with your health plan and your primary care physician prior to your visit.
If you have a health insurance plan that Great Lakes Ear, Nose and Throat Specialists participates with, you are responsible for co-pays, deductibles, co-insurance, and non-covered services. We will attempt to notify you of special services you may require that are not covered by your insurance company but ultimately it is the responsibility of the member to know their insurance benefits. We will be happy to provide you with any information you need to verify coverage.
If you have an insurance plan that Great Lakes, Ear, Nose and Throat Specialists does not have a contractual agreement with, you will be responsible for any and all fees at the time of service. We will be happy to supply you with a complete receipt for you to submit to your insurance company for reimbursement.
No Insurance
If you do not have health insurance, payment for services is expected at the time of your office visits. Please contact our office for an estimation of costs.
Referrals
It is the responsibility of the insurance member to obtain any referrals required by their insurance company and to update them as needed. If you do not have a current referral you might be required to reschedule your appointment.
Monthly Statements
Any outstanding balance is due upon receipt of a statement. If you have any questions regarding your statement, please contact our office.
Financing
We offer convenient payment plans through CareCredit for our patients. Speak with a provider at your appointment about payment plan options.
